I don't know about the ID Software case, but that ruby maker popup is just the standard ruby maker warning that you need to install a shared graphic pack relevant to the ruby maker. That pack is installed wherever and adds a regkey with a path to itself. The layering for that, seems to be identical to how it is for character-related graphics: in front of everything other than the gauges at the bottom, input-display, and menu. In the character-selection-menu, the screen seems to be brightened according to the transparency of the hovering weapons of certain characters, while those are supposed to be visible.
