For me, it appeared after upgrading my Fedora Core 6 from package wine-core-0.9.27-1.fc6.i386 to wine-core-0.9.29-1.fc6.i386. (All related packages were updated at the same time, so I'm not sure if it is wine-core or one of the related packages that causes the issue.) Currently, I only use wine to run Quicken 2000 deluxe and Quickbooks 6.0. On both applications, every time a new window is opened, the two leftmost menu options "go away". Once there are no more options, the menu bar disappears as well.
