I see there is a good reason to have executable MIME handlers since binfmt-misc is Linux-specific and not supported elsewhere. I like the idea of "don't have defaults that aren't explicitly specified" in general, but users might still not expect that explicitly opening a program with "Wine Windows Program Loader" will cause code execution, even if they get an "Open With..." prompt. I also like the idea of being able to say clearly whether a handler opens or executes a file, but am not sure how practical that is. It kind of feels like the MIME system itself is subtly broken here, at least now that we live in a world where app-specific sandboxing is a thing. It doesn't really hang, but takes a very long time to process queries. Even for this small file application makes ~1800 selectSingleNode() calls. This was implemented very inefficiently recently, but is now replaced with another implementation (see https://gitlab.winehq.org/wine/wine/-/merge_requests/10938) With current Wine application starts much faster for me. The apparent problem in that bug is that the rendering fails to update due to ddraw or else reasons, thus probably results in default window surface color showing up. The fact it changed to white maybe makes window more visible. Games such as - Guild Wars 1, Expedition 33 and Legacy of Kain Defiance Remastered to name a few. Didn't test other games but most likely affects them too. Issue started since 11.8 (and 11.9). Reverting to 11.7 games work fine. I tried the OP's patch with 11.9 release and the games started working fine again.
